Scholarship Application
Parents of students enrolled in a school owned and operated by the Roman Catholic Diocese of Phoenix are encouraged to apply to Catholic Tuition Organization for tuition assistance. Tuition assistance is awarded by Catholic Tuition Organization based primarily on financial need.
Application forms are provided and processed by Financial Aid Independent Review (FAIR) of Rosemount, Minnesota. FAIR application forms must be obtained from the eligible school where your student(s) attend. Students are required to be enrolled in grades K-12 prior to submitting an application for a scholarship.
Scholarship awards granted during the current school year are from the contributions received during the previous tax year.

Follow these Steps for Completing Your Application
Important Information for Filling out Scholarship Form
NOTE: This page is for information purposes only and cannot be used to apply for a scholarship. Original forms must be obtained by the school in which you are registered.
Applications must be obtained from the school where the student is registered. They are not available from the Internet and cannot be mailed to you from the Catholic Tuition Organization or Financial Aid Independent Review (FAIR).
Any student in grades K-12 who is enrolled in an eligible Catholic school in the Diocese of Phoenix can apply. Preschool students are not eligible to apply for a scholarship.
There is a $21.00 Application Processing Fee. If paying by check, make it payable to "FAIR", and enclose it along with your application and any required documents.
There are two methods of applying for a scholarship:
- Financial Need
- Fund Availability
Your application must be in the office of Financial Aid Independent Review (FAIR) by April 16th. If your child has enrolled and been accepted into a Catholic Tuition Organization school after April 16th, please contact the school for further information regarding applying for a scholarship.
Those meeting the April 16th deadline will be notified by mail no later than June 30th.
Financial Aid Independent Review (FAIR) is the company that processes the financial aid applications for the Catholic Tuition Organization. When mailing your application to FAIR, use the following address:

If you need assistance in completing your application, please call the school where your student is registered. Please do not call the Catholic Tuition Organization, as we will not be able to help you with specific information regarding your application.
I wanted to thank your entire staff and the board of directors at Catholic Tuition Organization for the generous awards for my children. We received nearly $6000 in assistance this year. We explained to our kids what a gift this is and how important it is to not take it for granted. If it were not for this help, they would never have been able to stay at St. Agnes.
