Scholarship Application
Attention CTODP Scholarship Applicants:
Thank you for applying to the Catholic Tuition Organization of the Diocese of Phoenix (CTODP). We do our very best to help make Catholic education affordable for your family.
The deadline for priority funding for the 2011/12 school year is April 18, 2011 (11:59pm).
Online Application
All CTODP applications must be submitted online at FAIR (School Code: 700/Password: ctodp700). Mail or fax your tax forms to FAIR: details online.
VISA or MasterCard Credit Card payment of $21.00 is required to complete the online application. If you do not have a credit card, you may purchase a VISA gift card at Wal-Mart or other retailers. Contact your School Finance Office if you have difficulties paying online.
PLEASE NOTE: Your child’s scholarship is generated by individuals/corporations who “redirect” their tax dollars to create tuition scholarships—giving costs taxpayers nothing out-of-pocket!
GOOD NEWS: It’s not too late to help INCREASE the funds we have for scholarships!
If you apply for a CTODP scholarship, we urge you to help us generate more funds for all applicants for the upcoming 2011/12 school year by doing the following:
- If you pay any Arizona income tax, you have until April 15th to give to CTODP and receive your tax credit—you get your money back when you file your 2010 AZ taxes.
- Ask your relatives, friends, neighbors and co-workers to do the same.
- Assist your school in promoting CTODP; help us raise more funds by April 15, 2011.
First-time donors are eligible to have their contribution matched dollar-for-dollar, meaning your gift could be doubled. To give or to learn more, visit www.catholictuition.org/match, or call 1-888-TAX-CRDT (888-829-2738). Thank you for your participation and support.

Any student in grades K-12 who is enrolled in an eligible Catholic school in the Diocese of Phoenix can apply. Preschool students are not eligible to apply for a scholarship.
Financial Aid Independent Review (FAIR) is the company that processes the financial aid applications for the Catholic Tuition Organization. When mailing your application to FAIR, use the following address:

I wanted to thank your entire staff and the board of directors at Catholic Tuition Organization for the generous awards for my children. We received nearly $6000 in assistance this year. We explained to our kids what a gift this is and how important it is to not take it for granted. If it were not for this help, they would never have been able to stay at St. Agnes.
